The ultimate goal of the program is to assist the individual in avoiding hospitalization, increase functioning level, and successfully integrating into the community. Clients are expected to gain motivation to maximize their potential, by learning to manage their mental health symptoms and how to live productively in a less restrictive and more independent living situation upon completion of the program. Each client will develop a treatment/service plan with a mental health counselor, and plan specific goals and objectives directly related to their needs and abilities. Each of the Long-Term Treatment Programs is presented in three Phases: Phase I – Orientation/ Stabilization, Phase II – Treatment and Recovery, Phase III –Community Re-Entry. Allowing for up to 18 months length of stay affords clients the opportunity to work with staff to understand, stabilize their psychiatric symptoms, manage their medications, and learn the necessary skills to integrate into the community successfully. Each phase represents important milestones in each of the client’s recovery and healthful living plan. Advancement to higher phases is conditional, individualized, and requires dedication and self-determination Click on site name for more information: |
