Services made available under this program may include:

• Individual counseling and psychotherapy
• Case management
• Assistance reaching out to and securing community resources
• Transportation assistance (i.e., assistance getting to appointments)
• Psychiatry and medication support
• Assistance with housing services to help locate suitable living spaces for clients
• Assistance getting financial and health benefits
• Substance abuse treatment
• Help identifying and accessing educational and/or employment opportunities

Basic Criteria

• Adults 26 -59 diagnosed with serious mental illness
• Must have prior hospitalizations or institutionalization and/or be homeless
• Must be referred by the Department of Mental Health
• If referred by another agency, please call to see how you can get referred by us to the Department of Mental Health to get enrolled in FSP
• Call for a no cost assessment to see if you qualify for services
