Services made available under this program may include one or more of the following:

• 24-hour care and supervision (24- hour medical care is not available)
• Assistance with Self-Administration of Medication
• Mental health services (treatment planning, therapy both group and individual)
• Family Support
• Substance Abuse Recovery Support
• Peer Mentoring
• Social Rehabilitation (daily living skills, independent living skills, leisure and occupational activities)
• Community Linkage
• Crisis Management
• Case Management
• Vocational & Educational assistance
• Housing and Benefits assistance

Basic Criteria

• Ages 18 and over
• Mental health and/or co-occurring disorders diagnosis
• A desire to participate in a program designed to address mental health needs (this is not a stand-alone housing program)
• Must be ambulatory based on program license and medically clear (call us, if will help you with this)
